Strut for Mutts Registration Form

Please fill out this form completely to register for the Strut for Mutts Walk being held on June 6th, 2010. We must have a form for each person walking.

We look forward to seeing you! Please tell all your friends and family about this walk. The more people we have walking, the more pledges we get and the more money that goes towards our rescue dogs. If you haven't paid your registration fee yet, please bring it with you to the walk. No t-shirts are given out until registration fee has been paid.

Failure to send in the registration fee BEFORE MAY 15th will result in no t-shirt being ordered for you but you can still collect pledges and walk with us.

IF you have a tee shirt from  previous year, you're more than welcome to wear that shirt and the $20.00 will be a donation to Mini Mutts Rescue.
